5/4/2023 EnchantaVaughn Approved (AM)Upon arrival, client exhibited low rapport with his teacher and peers. Client seemed like he didn’t want to be in school. Client was dressed in appropriate clothing with near braids indicating he was prepared to come to school. It may have been a small situation that happened between leaving the home and coming into class by the way client looked. Client displayed restlessness and was unfocused during morning leisure activities, especially during class time and encore electives. Client expressed he was running late to school and didn’t get a chance to eat breakfast. Client displayed hyperactive behaviors and was required to be redirected over 5 times as reported by his instructor and counselor. Client appeared guarded and defensive as observed by QMHP. Client was observed talking back to his teachers in a disrespectful manner as he was being reprimanded. (PM) Client was prompted to refocus 3-4 times by QMHP. Client was reluctant and hesitant to follow rules and stay with his age group. Client kept getting off task and involving himself in miscellaneous acts not directed by staff. Client got in trouble for leaving his assigned area without permission. Client continued to require 2-3 prompts of redirection by QMHP and other adult staff. Client refused to do what he was asked and QMHP pulled him to the side for one on one. Client presents anger and frustration at this time. Client seem to be irritated and needed a second to cool off before processing. Client was defiant of QMHPs prompts 4-5 times. Client blamed others for his negative behaviors and appeared unapologetic for participating in disruptive actions. Client was not able to reduce attention-seeking behaviors or follow directions. Client was reported to have struggled to maintain expectations while in class. Client was disruptive and defiant while in the hallways, as reported by adult staff. Client was required to be redirected 5-6 times prior to QMHP coming to the session area to intervene. (EOD)Client stated the instructor was yelling at him for no reason. Client was reminded to be respectful and try to express healthier emotions even when he’s frustrated so that we can figure out what’s going on without him lashing out and making it hard to help him with his issues in school. Client was receptive of this advice and made some changes before going back to class. However, client did not meet ISP objectives as evidenced by the client being loud and disruptive towards all staff throughout the bulk of his day and educational hours.
